Just cliped through the wall where you do the WESS to get to kakariko after escape. Wat
I had that happen too and when I asked about it, it was already known/useless. ;)
I only had it happen once, so I guess once you get good at the WESS you do not need to worry about it happening randomly.

It happens when you take the angle too sharply and you will clip through
